Sports Physicals for 2010-2011 School Year Start April 1.
Sports physicals for the new school year may be performed on or after APRIL 1, 2010. You can schedule an appointment with your family physician or at the Wellness Center (651-2100).
All forms must be completed before you make an appointment. Please fill out the DIAA Sports Physical Forms. If your child is not registered at the Wellness Center yet, please complete the Consent and Registration forms.
